Upwards of 20 million kids face serious illness and disability. Our unique multiyear program signs children onto college athletic teams across the country, and based on a clinical model, ensures each has the personalized game plan they need to win – effectively building confidence and independence.

TEAM IMPACT CASE MANAGEMENT
Our Case Managers, all masters-level social workers and child life specialists facilitate relationships between kids and their teams, ensuring that they build rapport, connection, and trust.
PERSONALIZED GOAL SETTING
Case Managers work with families to identify personalized goals and develop a plan to achieve them.
CLINICAL INTERVENTIONS
Case Managers use a range of therapeutic techniques, including active listening, validation, scripting, reflection, and anticipatory guidance.
ENHANCED SUPPORT INITIATIVES
Case Managers provide individualized education and support to teams about topics including self-care and coping with grief and loss.
BUILDING RELATIONSHIPS
Team IMPACT is a year-round experience. Through a structured approach, a clinically-trained Team IMPACT Case Manager monitors, guides, and supports both the families and teams to ensure meaningful bonds are formed over the course of the relationship.
